Relevance to General Practice

End-of-life care is a significant role of general practice with most patients with a progressive life limiting illness wishing to be cared for and, if possible, to die at home. This care inevitably requires the input of a GP with support from other community based services such as a palliative care team and community nurses. Supporting and encouraging GPs to offer or continue providing palliative care is not only important for individual patients and their families but for the health system and the whole community. This on-line End of Life Care module aims to build the capacity and skills of general practitioners in caring for adults with a progressive life limiting illness and provides a framework for patient ‘shared care’ with Specialist Palliative Care Services in metropolitan and peri-urban Adelaide.

  • Review an Advanced Care Directive and Resuscitation Plan 7 Step Pathway for end of life care to enhance the safety and quality of patient care
