GPs are keeping patients on sub optimal levels of basal insulin even though patients are no longer reaching target HbA1c. Their reluctance to titrate centres around patient concerns around increased risk of hypos at higher insulin doses. Guidelines recommend titrating basal insulin to appropriate insulin doses or moving to insulin if targets are not being met. This educational module covers the treatment options available for patients with type 2 diabetes, managing patients who have uncontrolled type 2 diabetes despite insulin use, and stresses the importance of achieving glycaemic targets.

Relevance to General Practice

An estimated 1.7 million Australians have diabetes, with type 2 diabetes (T2D) accounting for 85% of all diabetes. Data from the Australian Institute of Health and Welfare indicates that diabetes was the underlying cause of 3% of all deaths and an underlying or associated cause of 10% of all deaths in 2011. Undiagnosed and poorly sub-optimally managed T2D can lead to a range of complications, including stroke, coronary artery disease, renal failure, and blindness, making the early identification and optimal management of people with T2D critical. Results from the General practice activity in Australia 2014-15 report suggest that 5 million non-gestational diabetes encounters were managed in Australian general practice in the year 2014-15, making it the third most frequently managed chronic medical condition. GPs are therefore well placed to play a critical and prominent role in the optimisation of T2D management.

  • Review the patient-mediated barriers to initiation of insulin and titration of insulin doses
  • Identify patients who might benefit from switching insulin to reduce the risk of hypoglycaemic episodes
  • Implement simple dose titration strategies with ptns to attain a maintenance dose that will facilitate good glycaemic ctrl while minimising hypoglycaemic episod
  • Identify patients who would benefit from referral to an endocrinologist
