A large proportion of patients presenting with gastric cancer have disease beyond locoregional areas. These are incurable. This is especially so in Western countries, where the rate of disease is so low that screening programs are not cost-effective. Much higher rates of earlier gastric cancer are identified in countries of high incidence where screening programs are undertaken, such as Korea, Japan and parts of South America. It is therefore necessary that treatments are both palliative and curative depending on disease stage.
