A miscarriage is the spontaneous loss of a pregnancy in the first 20 weeks of pregnancy. 
Early pregnancy loss is defined as a nonviable, intrauterine pregnancy with either an empty gestational sac or a gestational sac containing an embryo or fetus without cardiac activity in the first 12 weeks and six days of gestation. 
In most cases when a pregnancy fails, it is because of a problem with the embryro-chromosomal, developmental, or both. Pregnancy loss is almost never caused by anything the woman did or failed to do.
