To enhance GP knowledge and confidence & increase a multidisciplinary approach to managing Type 2 diabetes within their general practice, by utilising case conferencing with specialists (Endocrinologist & Diabetes Educator).

Relevance to General Practice

The Hunter Diabetes Alliance developed "in-house" clinics for general practice, as a vehicle to support improved collaboration and integration of clinicians in the Hunter, particularly in regards to standardisation of Diabetes care being provided, communication between clinicians and patients, and access to skilled diabetes care providers. In addition, it is recognised that "patient activation" is essential to empowering patients with Type 2 DM to manage their disease,1 but that this is not currently routinely measured in primary care. Much literature exists to suggest that increasing the capacity of GP coordinated multidisciplinary services to prevent and manage diabetes and its complications results in improved patient satisfaction and clinical outcomes.2,3,4 However, interviews with local multidisciplinary clinicians, from both the private and public sector to date have identified a lack of knowledge between clinician groups of the barriers they face

Learning outcomes
  • 1. Identify opportunities for process redesign or clinical, quality or safety improvement in the management of patients with Type 2 Diabetes.
  • Identify diabetic emergencies and options for early intervention, to improve clinical outcomes.
  • Describe the local process required to escalate care for all diabetic patients in order to ensure optimal clinical outcomes.
