This workshop aims to update GPs who refer to East Metropolitan Health Service (EMHS) hospitals with a focus on older adults. Discussions will be on the latest assessment and management of a range of common conditions and pathways for referral using presentations, case study discussions, quizzes and a workshop to improve the way we work together to deliver optimal care.Relevance to General Practice
EMHS is committed to improving the interface between GPs and hospital services to enhance patient care. Awareness of the available interventions and services will lead to earlier intervention and improved transition of care. The Sustainable Health Review noted the importance of seamless transition of care between general practice and hospital settings.(1) Integrated Care is a key concept in contemporary health service delivery; upskilling GPs in processes that lead to improved patient care and patient experience is timely(2). One of these is Advance Care Planning, for which uptake is lower in WA than in other states (3).
