Primary care education will enhance general practitioners’ knowledge of Chronic Kidney Disease (CKD), its associated risk factors, co-morbidities and appropriate management. In addition, the shared care model will improve coordination of services across primary and secondary care whilst also improving the access for integrated care closer to home for people with CKD in Caboolture.

Relevance to General Practice

Data highlights that 10% of people attending general practice have CKD, but most do not know it. Similarly, every second patient that visits his or her general practitioner with type 2 diabetes will also have CKD

Learning outcomes
  • 1. Identify skills in early detection, investigation and management of CKD
  • 2. Implement a practice-based system, for patient safety, to identify patients at higher risk of CKD for a kidney health check
  • 3. Integrate GP Management Plans, Team Care Arrangements and Home Medicine Reviews in conjunction with Kidney Health Service, Metro North HHS.
