The Flinders Program for Chronic Condition Management 2-day face-to-face workshop addresses the needs of GPs and the primary care workforce to develop the necessary skills to support chronic condition self-management. Participants will learn the Flinders Program™ tools and processes that provide a framework enabling practitioners and clients to: a) undertake a structured assessment of self-management, b) collaborate in identifying problems and goals, leading to c) the development of an individualised care plan. Participants will also explore potential barriers and enablers to implementation of the Flinders Program into clinical practice.

Relevance to General Practice

GPs are at the frontline of the prevention and management of chronic conditions. The Flinders Chronic Condition Management Program™ incorporates the core skills needed for GPs and the primary care workforce to better support clients with chronic conditions. It is an evidence-based program that is consistent with the Chronic Care Model developed by Wagner et al. (1996). The set of tools that form an integral part of the program has been tested and evaluated with a number of different patient populations both within Australia and internationally.
