83614 - Patients at risk of falls Educational visit program - iSOLVE Program

The educational visit session is provided as part of the iSOLVE randomised controlled trial to test processes and pathways involving identifying patients at risk of falls, multifactorial risk assessment, recommendation of fall prevention management tailored to each patient, and referral to relevant falls prevention service provider. The education facilitator will organise a one hour session at a time. The education session may be delivered to individual GPs in separate occasions or to a group of GPs jointly, depending on their preference.

Relevance to General Practice

The educational visit session is tarted at GPs to train GPs in fall prevention and implementation of fall prevention screening, assessment and management in general practice and the primary care setting. Falls-related injuries accounted for the majority of all hospitalised injury cases among people aged 65 and over (Aust Institute of Health and Welfare). Hospitalisation rates in Northern Sydney have been consistently higher than the NSW average. Falls can be prevented. There is strong evidence that balance and strength exercises, home safety interventions and medication reviews are effective in preventing falls in older people. GPs and general practice staff play an important role in promoting fall prevention in the community as the primary care providers. If GPs routinely identified patients at risk of falls, they could develop care plans targeted at falls prevention.
