Relevance to General Practice

Involving GPs in fall prevention. General practitioners (GPs) and general practice staff play an important role in promoting fall prevention in the community as the primary care providers.9-11 If GPs routinely identified patients at risk of falls, they could develop care plans targeting at fall prevention.10,12 However, research has shown various barriers for uptake of evidence relating to fall prevention in the primary care setting. (Refer to attached supporting documentation for more information.)

Learning outcomes
  • Implement a systematic approach to identify older patients (65 years or older) who are at risk of falling
  • Prompt patients to recognise their fall risk and discuss what the patients can do to prevent falls.
  • Assess patient’s risk of falling and determine tailored fall prevention recommendation according to patient’s fall risk factor
  • Refer to fall prevention service providers in the community to improve uptake of fall prevention interventions.
  • Review and encourage patient’s uptake of fall prevention recommendation
