The purpose of this webinar is to enhance the knowledge, skills and confidence of the primary care workforce to promote, initiate and continue discussions regarding future health care preferences in routine care. 
Using a case study, there will be a facilitated, interactive panel of general practice staff discussing what advance care planning is and its benefits to patients, families and health professionals. It will identify opportunities to initiate and continue discussions on advance care planning and describe the various roles of primary care staff in this area

The older population in Australia is growing with people living longer and dying an expected death from chronic conditions. General practice is caring for people in the community with advanced chronic conditions that will soon lead to death. 
As people live with complex, chronic life-limiting diseases they will have increasing interactions with the health system including episodes where treatment options need to be considered and decided upon. It is vital to know a person’s wishes to ensure the system is providing person centered care. 
Person centered care takes into account the person’s right to determine their own health care priorities. Advance Care Planning improves quality of life. It has been shown to reduce anxiety, improve management of chronic disease and quality of life
