Relevance to General Practice

Continuity of care is a central to the RACGP’s view of high-quality care,1 and has been shown to be associated with improved health outcomes.2 However, the traditional model of healthcare, where patients receive ongoing care from one practitioner, is now no longer the norm. There are a variety of reasons for this change, including the increasing number of people living with chronic diseases and GPs now tending to work in larger practices, so that patients will not necessarily always see their preferred practitioner. Changes in technology have also produced new ways of providing healthcare, such as computer-based options. It is important to consider the issues and challenges surrounding continuity of care and how this can be maintained within the current health system. Equally important is the need to be aware of technological developments that can be used to improve continuity of care and ensure ongoing high-quality healthcare, particularly in an ageing population.
