Chronic and Complex Care
This unit of study explores the impetus for change in conventional health service delivery and the issues related to providing health services for people with chronic disease. Nurses manage patients with chronic and complex issues and, in doing so, need to develop their knowledge and skills in this area. Students explore issues in the management of clients in their care by critiquing appropriate models of care, patient assessment and management. Themes in this unit of study refer to epidemiology, models of care, collaborative case management, assessment (principles and application), patient management, disease progression, medication management, enhanced primary care, as well as bioethical and psychosocial issues.

Theme 1: Landscapes and chronic illness
Appraise the evidence-based policies, models and guidelines that inform chronic care management in Australia
Theme 2: Models of care
Explore the impact of the illness trajectory on the person experiencing chronic health issues
Theme 3: Nursing assessment
Evaluate the needs of the person with chronic illness to develop strategies that are person-centred and holistic
Theme 4: Nursing support
Discuss the chronic health issues experienced by Aboriginal and Torres Strait Islander and culturally and linguistically diverse communities
Theme 5: Engaged patients
Examine the role of the nurse and integrated support services in providing person-centred chronic care management
Theme 6: Experiencing chronic illness
Analyse the health literacy and communication skills needed by individuals to increase capacity in self-management of chronic care conditions
