Diagnosis and Management of Chronic Heart Failure in General Practice

Heart failure is a significant health issue in Australia, with complex diagnostic and management methods that must be tailored to each patient. This ALM comprehensively covers the many stages of heart failure management including the identification of risk factors, diagnosis and investigations, pharmacological and non-pharmacological management, management of progression and comorbidities, the use of implantable devices and end-of-life care, and multidisciplinary care programs. This activity is worth 40 RACGP QI&CPD or 30 ACRRM PDP points.

By completing this learning activity, participants will be able to:

  • Understand the aetiology, risk factors, signs and symptoms of chronic heart failure (CHF).
  • Know how to investigate CHF in order to determine an accurate diagnosis and prognosis for optimal management.
  • Be familiar with current evidence-based recommendations for the pharmacological treatment of CHF.
  • Understand the importance of non-pharmacological approaches to CHF management, particularly nurse-led patient education to encourage self-management.
  • Understand how general-practice led models of care that focus on co-ordination of AHP and specialist services may improve outcomes for CHF patients.
