Patient’s lives and those of their families and carers can be overwhelmingly challenged by the consequences of injury, surgery, illness and debilitation. This program includes:

    Need for Program

    Whilst acute care stays for people in hospital are often days or weeks, nurses are now increasingly encouraged to use this time to begin the recovery “back to the community” process. This includes preparing them for the care that will continue after discharge to maximise quality of life - in the long term. Promoting wellness and self-care using up-to-date knowledge and techniques can reduce the burden of ongoing disability in the community. These are key nursing practices that aim to prevent the revolving door of hospital re-admission.

    Purpose of Program

    The purpose of this program it to provide nurses with knowledge, skills and practical information to guide, and direct their patients to live a better quality of life after hospitalisation.

  • Patients with conditions that are known to cause long term disabilities will receive interventions or activities that will benefit them on discharge
  • Use effective communication skills with members of the interdisciplinary team to ensure advocacy and best patient outcomes
