Living Longer, Dying Better: A Framework For Palliative Care In Community-based Aged Care

This online module has been designed to increase GPsÂ’ capacity to manage the care of older Australians with advanced progressive chronic conditions living in the community including private homes and residential aged care facilities (RACFs).
In Australia, the demographics of death and the processes preceding it are changing. People are living longer often with multiple chronic conditions and most die expected, rather than sudden, deaths as a result of these conditions. The majority of people report that they would like to die at home, although most die in hospital.
Given that most people die an expected death, it stands to reason that death can be planned for and required care delivered in a pre-emptive fashion.
GPs are optimally placed to deliver this care and are central to its success.
