Transition to Practice Program (TPP)
The Transition to Practice Program (TPP) by the Australian Primary Health Care Nurses Association (APNA) is a nationally recognized initiative designed to support nurses transitioning into primary health care roles. Whether newly graduated or experienced nurses entering general practice, aged care, or community health, TPP provides a comprehensive framework to build confidence and competence. The program includes access to exclusive education modules, such as the Foundations of Aged Care or General Practice Workbooks, and structured self-assessments to guide CPD planning. Participants receive mentorship from experienced nurses through small-group learning and join a Community of Practice for peer support. Induction workshops and ongoing guidance from APNA staff ensure a supportive learning environment. The program is funded by the Department of Health and Aged Care or Primary Health Networks, making it accessible to eligible nurses at no cost. TPP aims to improve nurse retention, enhance clinical capability, and foster leadership in primary health care settings.
