This session will describe the different types of post-diagnostic support available to people following a dementia diagnosis and the role of primary care practitioners in suppporting people to live well.

Learning Objectives

By the end of this session you will be able to:

  • Describe the principles of psychosocial approaches used to enhance the well-being of people with dementia and support people with dementia to access local services and referral pathways including voluntary and community services
  • List strategies to support medication management and adherence to pharmacological interventions
  • Explain the driving guidelines specific to dementia
  • Discuss methods to assess a carer’s psychological and practical needs and the relevant support available

A variety of types of post-diagnostic support are available to people with a diagnosis of dementia, ranging from providing information, signposting and advanced care planning to providing or referring to structured interventions.
