Chronic illness care requires a structured, integrated and multidisciplinary management plan. This workshop aims to improve chronic disease management, streamline the monitoring of clinical parameters and incorporate preventative healthcare and lifestyle advice.
As part of practitioner management for people with chronic disease, the care General Practice model should:
• Advocate patient self-management
• Include extended and planned care
• Work in partnership with a multidisciplinary care team
• Schedule systematic and routine follow up and review
Specialist presenters will explore a broad range of chronic conditions including pain management, obesity, neurological conditions and chronic cardiac conditions
General practice plays a crucial role in the diagnosis and care of those suffering from chronic conditions, as primary care practitioners are the patient’s first point of contact with the medical profession.
Patients with chronic conditions often present with lengthy and complex histories including multiple physical illnesses and psychosocial issues. As a result, accurate diagnosis may prove challenging due to the diverse range of presentations.
