Healthy Communities Foundation Australia Ltd was established in 2001 as a registered charity to address the critical shortage of health and hospital services in rural areas. It was founded by a group of rural GPs and communities in north-western NSW determined to improve access to care in their region.

Since then, we've supported more than 73 communities across Australia. Our work focuses on our patients, consumers and the communities where they live. We design and deliver hybrid primary healthcare services for people where and when they need them.
