As a contracted Nurse Practitioner you will assess, diagnose, and manage a diverse caseload via telehealth platforms. You will collaborate with GPs, allied health professionals, and nursing staff to provide safe, evidence based care. Key responsibilities include virtual consultations, developing care plans, prescribing within scope, documenting accurately, and supporting practice improvement initiatives.

• Conduct comprehensive telehealth assessments and consultations for primary care and targeted specialties.
• Formulate and implement management plans, including prescribing and referrals when required.
• Perform risk assessment, chronic disease management, and acute care triage through virtual channels.
• Maintain detailed clinical records and ensure compliance with regulatory and organizational standards.
• Participate in multidisciplinary case discussions, quality improvement activities, and practice development.
• Provide education and support to patients and families to promote self management and health literacy.
• Uphold patient privacy, telehealth best practices, and digital documentation protocols.

Competitive hourly remuneration of $120 to $150 based on experience and qualifications, flexible start timeframe, and the ability to work remotely with a supportive Melbourne based team. • Endorsed Nurse Practitioner or Registered Nurse with endorsement status where applicable.
