Clinical Pharmacist
HPS and its network of pharmacies supply a wide range of pharmacy services to health facilities, including private hospitals, public hospitals, cancer centres, correctional facilities and specialist clinics.

About the role
An exciting opportunity has become available a dynamic and enthusiastic pharmacist to work with our team at HPS Pharmacies - Knox. This role will require the pharmacist to work Monday to Friday 9am to 2pm.
The role is to cover Clinical Pharmacist duties and on-the-job training will be provided to the successful applicant. It is a fantastic opportunity to develop in clinical pharmacy skills across a number of disciplines. Community pharmacists are encouraged to apply, as this is a great pathway to transition into hospital pharmacy.
Please note, there is a requirement to participate in an on-call roster. This role is also available for an immediate start.
Key responsibilities

Dispense and supply medications in line with legal and professional requirements.

Take Medication History on admission and undertake Medication Reconciliation.

Conduct inpatient medication chart review.

Coordinate patient medication requirements for inpatient stay and for discharge.

Prepare discharge medication profiles.

Liaise with the relevant medical staff as needed.
You must have

A can-do attitude and team focus.

Self-motivation and the ability to work independently.

Pharmacy experience.

Oral and written communication skills to effectively interact with all levels of hospital staff.

A high level of attention to detail.

The ability to organise tasks and solve problems.

Current registration with the Pharmacy Board of Australia.

Membership with a Professional Indemnity Insurance provider.

Hospital experience will be highly regarded but is not essential.

Current Covid-19 vaccinations as per Public Health Orders.
