🩺 About the Role

We are seeking a compassionate and collaborative General Practitioner to join our growing team of Wellness Partner Clinicians. This role plays a vital part in improving medication safety and clinical outcomes for at-risk older individuals through direct partnership with our credentialed pharmacists and geriatricians.

You will be primarily responsible for:

• Conducting patient assessments and clinical reviews to support safe, appropriate prescribing.

• Generating referrals and clinical input for Home Medicines Reviews (MBS 900) and Residential Medication Management Reviews (MBS 903).

• Participating in clinical case conferencing with credentialed pharmacists, geriatricians, nurses and patients.

• Supporting the multidisciplinary team in managing chronic disease, deprescribing, and optimising pharmacotherapy for vulnerable populations.

• Providing continuity of care for aged care residents, both in the community and residential settings, with a focus on those experiencing barriers to traditional healthcare access.

• A national network of credentialed pharmacists, embedded in aged care and community settings.

• Internal and external geriatricians, psychologists, and social support professionals.

✅ About You

To succeed in this role, you will need:

• Current AHPRA registration as a General Practitioner (General or Specialist).

• Eligibility for a Medicare Provider Number (ability to bill MBS required).

• Strong commitment to medication safety, quality use of medicines, and multidisciplinary care.

• Experience working with older adults or in aged care (desirable but not essential) and with other at-risk individuals.

• An interest in interdisciplinary teamwork and contributing to a mission-driven organisation.
