Salary: $255,000 + salary packaging (FT) or $145/hour (PT)

About Us

Wellness Partners Foundation is a not-for-profit organisation addressing inequities in health and wellness for Australia’s most at-risk individuals.  We are pioneering new ways to deliver care to those that need it most.  We are now recruiting our first General Practitioner – a trailblazer who will help shape an innovative healthcare model, working hand-in-hand with clinical pharmacists and our Foundation’s support teams.

The Role

This is not a typical GP job. You’ll deliver telehealth primary care to some of the most vulnerable groups in Australia. You will:

• Provide accessible, telehealth GP services to individuals facing barriers to care.

• Collaborate with clinical pharmacists to strengthen medication safety and continuity of care.

• Support medication review referral pathways for residential and community care.

• Act as a clinical supervisor for pharmacist prescriber training programs with our university partners.

• Contribute to harm reduction initiatives in areas such as addiction, medication adherence, and wellness promotion.

• Participate in clinical governance functions and Medication Advisory Committee meetings.

• Support transition of care stewardship and medication continuity.

• AHPRA registered GP with a passion for equity in health.

• Committed to serving at-risk individuals, including those who are homeless, older, rural, First Nations, or experiencing addiction.
