GP Practice Nurse

About the Role:
Previous experience working as a Practice Nurse in a Family Medical Clinic is essential.

We are an expanding practice that requires an experienced Practice Nurse who enjoys Chronic Disease Management and has Immunisation certification. You will work in a team which supports GP's and their patients to optimise their chronic conditions.

Essential Skills:

• Ensure all patients and visitors are received in a friendly and helpful manner, either in person or over the phone.

• Enjoys Chronic Care Nursing.

• Immunisation Certificate must be held at the time of application

• Has the clinical skills to conduct detailed health assessments to improve patient health outcomes

• Identify, assess and develop chronic disease management care plans that optimises improved health outcomes

• Has a passion for managing, educating and empowering patients on preventative health care.

• Promote preventive screening

• Best Practice software experience is preferred, with training provided for successful candidates

• Maintaining demographic and clinical data in patient files often using software like Polar Explorer.

• Maintain a high level of accuracy when recording and maintaining patient and clinical information

• Identify and provide physical and emotional patient care in your role

• Adhere to RACGP 5th Edition Standards and AGPAL Accreditation requirements

Ear syringe - trained

Phlebotomy - trained

Wound care

Maintain Infection Control standards
Cold Chain Management

Essential Requirements:

• Minimum 2 years of Practice Nurse experience in a Family Medical Clinic is required. (Please don't apply if you have not worked in a Medical Centre as a Practice Nurse.)

• Registered Nurse - Division 1/Enrolled nurse - Division 2

• Current AHPRA Registered
