Chronic Care Australia is currently looking for a talented new Level 2 physiotherapist to join our young and diverse team. Chronic Care Australia is a globally awarded company that is leading the way in value based primary healthcare delivery with innovative new exercise medicine programs for varied chronic conditions.

The successful applicant will use their excellent physiotherapy skills to manage and deliver best care to a variety of general and complex patient cohorts.

The clinical case load will include both individual physiotherapy, clinical pilates and group supervision of our Care Clinic as well as involvement in our real world research pilots. These pilots involve working in best practice multidisciplinary teams (MDTs or integrated practice units, IPUs) around specific chronic conditions. See our current active pilots here. The successful applicant will have the opportunity to further develop their clinical skillset by liaising and working with our IPU clinical leads in areas of interest to them. There is also opportunity for project based, non-facing opportunities for the right candidate.

To be successful in this role you will need:

  • Minimum 2 years’ experience in assessing and managing clients with a range of conditions, including exercise prescription

  • Passion for quality and value-based care whilst being able to work in a high functioning, innovative and dynamic environment

  • Demonstrated experience in delivering exercise rehabilitation and effective education

  • Experience working collaboratively in multidisciplinary teams

  • Demonstrated experience or willingness to evolve in quality improvement and/or real-world research and data collection
