Aged Care GP is a growing organisation dedicated to providing GP medical care to residents of residential aged care facilities.
We believe passionately that all older people deserve access to exceptional medical care, to be seen as individuals with unique physical and emotional needs, and to be empowered to make as many decisions as possible. We believe that older people should be surrounded with respect, understanding and love, and to be given the opportunity to play an active role in their medical care.
As a GP working with Aged Care GP you’ll work in partnership with aged care facilities and work closely with their multidisciplinary teams. You will carry out comprehensive medical assessments, contribute to care plans and case conferences, collaborate with pharmacists on medication management, and ensure comprehensive handovers take place.
Providing dedicated GPs to residential aged care facilities enables long term relationships to develop with residents, a reliable and consistent service, high continuity of care, and an alleviation of stress for facilities, residents and their loved ones.
Working with Aged Care GP will enable you to have greater influence in the care you provide and tailor your style of care and interaction. You will have the flexibility and time to adapt your approach for different patients and make a real difference in their lives
