Relevance to General Practice

Implementation of the PCMH Model of Care is being trialed in Australia to transform the way primary health care is delivered to improve patient outcomes and focus on sustainable cost. There is strong evidence to prove that patients who have a continuous relationship with their GP and home practice receive higher quality preventative care and have better outcomes. 
Building a high performing general practice will improve access to health services, team care and decrease unnecessary presentations to emergency departments. This training focuses on the importance of a team-based approach to patient care to improve patient outcomes and experience.
