You will join our growing service based in our new $90million Residential, Health & Wellbeing facilities in Windsor, Caulfield and Carnegie.
About the Role
The Active Living Services team at Jewish Care Victoria operate Community Aged Care Services which includes a multidisciplinary Allied Health Team. We require a Senior Physiotherapist to support and consult with our aged care clients with Home Care Packages and CHSP funding. Physiotherapy services will be provided ‘in home’, ‘in rooms’ and in group programs with a focus on independence and living actively.
As part of the Allied Health Team you will provide support to individuals and groups using an evidence-based approach. You will work closely with Case Managers and referrers and provide responsive, comprehensive assessments making sound recommendations to ensure best client outcomes.
The Allied Health Service will provide assistance and support, referral to relevant community-based resources and effectively link people to ongoing services to overcome a wide range of health concerns.
Skills and Experience
- Degree in Physiotherapy with current AHPRA registration.
- At least three years’ experience working as a Physiotherapist in Aged care or Health Care.
- Demonstrated knowledge of a Wellness Reablement approach to service delivery in the community aged care.
- Demonstrated experience and well developed clinical skills whilst working within community aged care.
- Demonstrated ability to work as part of a team as well as autonomously
- Experience in the development, facilitation and evaluation of group programs
- Excellent customer service skills and the ability to establish genuine rapport with services users and colleagues.
- Passion and care to help a diverse range of people.
