What You Will Be Doing
We are seeking an experienced, innovative and flexible Physiotherapist to work as a member of the Transition Care Team. This is an exciting, challenging and rewarding position, suited to a Physiotherapist with a commitment to Best Practice in Integrated Care.
The Transition Care Program is a jointly funded DHS/Commonwealth Program designed to assist elderly patients transfer from their ‘inpatient hospital stay’ either to their home or to residential care. The aims of the program are to assist these patients attain their maximal functioning capacity and to help determine their long term care arrangements.
What You Need
The successful applicant will need to be highly motivated, innovative and enthusiastic and to be able to work both independently and as part of a team.
The successful applicant will rotate across both the Transition Care Residential program and the Transition Care Home based program on a 6 monthly basis. The successful applicant will also be involved in the supervision of Allied Health Assistants.
- Experience in one or more aspects of Health or Community Care is essential.
- Registration with AHPRA.

|Selection Criteria||Essential: • Registration with the National Physiotherapy Registration Board (AHPRA)
Essential: • Demonstrated experience as a Physiotherapist in equivalent work settings
Essential: • Capacity to supervise students, and Allied Health Assistants and promote professional competence and foster individual staff development
Desirable: • Knowledge of/experience in Sub-Acute service provision.
Essential: • Demonstrated competence in Orthopaedic / aged care physiotherapy
