About the Community Health Nurse role

You'll join the medical services team, working alongside General Practitioners, Nurses, support staff and our wider multi-disciplinary service to provide comprehensive primary health care to our community. Utilising your experience in a General Practice setting, you'll undertaking clinical duties to plan and coordinate client care, and assist with clinical procedures as required.

Due to an internal secondment, this is a 0.5FTE fixed term position until December 2024 and will report to the Clinical Nurse Coordinator.

Your Passion and Commitment will see you:

  • Perform clinical duties such as triage, preventative screening, immunisation, wound management chronic disease management, and assist GPs as required,
  • Plan and coordinate patient care, care plans and health assessments
  • Use health literacy principles to lead patient education, community health and outreach programs 
  • Consult with other medical and multi-disciplinary staff or providers, and act as an advocate for clients

To succeed in this role you will have:

  • AHPRA Registration as a Registered Nurse (Division 1)
  • Demonstrated experience in a General Practice setting (At least 1-2 years preferred)
  • Willingness to learn and work flexibly in a dynamic environment
  • Excellent interpersonal and communication skills, including problem solving
  • Familiarity with recall and reminder, data and clinical software systems
