Salary: $75,000 - $100,000 + super + travel allowance

Roles & Responsibilities:

Our Clients vision and mission is centred on delivering exceptional quality care within the Aged Care and Disability Sectors. They provide various community and in-home support services catered towards individual needs. 

 

They are currently seeking a Physiotherapist to service the Melbourne Community through home-care packages.You will be working within a multidisciplinary team to offer the ultimate care to your participants. Providing in-home care and building relationships with participants and their families during a flexible work-week. This role is open to New Graduates as well as Senior Clinicians.

 

As a Physio you are expected to:

  • Provide high quality Physiotherapy assessments and intervention for consumers within the context of Aged Care in a professional, confidential and culturally safe manner, in keeping with best practice standards
  • Completing consumer focused Physiotherapy assessments to manage a range of musculoskeletal, neurological, cardio respiratory disorders.
  • Ensure integrated service delivery and follow up, coordinating and communicating as required
  • Incorporate a holistic approach to health care that incorporates health promotion activities to consumers according to their priorities and requirements
  • Ensure information is recorded accurately and in a timely manner in the Management system and that records are maintained in accordance with the Aged Care Quality Standards and company policies and procedures
  • Ensure privacy and confidentiality is adhered to in relation to consumer’s information
  • Providing manual handling and education training for carers and family members
  • Consulting with site staff, doctors, consumer, family and other care professionals to develop consumer care plans, goals, nature of interventions and plan for review
  • Regularly reviewing consumers outcomes and make any necessary changes to align with consumer goals
  • Documenting each consumer treatment and general care in clinical notes as per company guidelines
  • As Attending and participating in meetings, including multidisciplinary, staff and management meetings on site and as directed by your Manager

Essential Criteria:

  • Excellent communication and organisational skills
  • Current Working With Children Check
  • Valid Australian working rights
  • Full Registration with AHPRA
  • Vaccination in line with government mandates
  • Have your own car and valid driver's license
